Marital Status and Name Change Help

Regardless of whether either spouse changes her/his name, employers of both the husband and wife should be informed of the new spouse.  
Even if there is no intention of extending benefits to the new spouse, it makes locating the next of kin much easier in case of an emergency.  
Certain retirement programs are also influenced by marital status.

Likewise hospitals, doctors, dentists, etc. should be informed of a new spouse in case there is an emergency.




There are links below to many forms to help you accomplish getting your name changed with the numerous parties who will need the new information.  

Some of these parties will want a
certified copy of your marriage certificate.  You will receive one certified copy after your marriage has been officially
recorded in the Florida county where you were married.  Additional copies may be ordered from the State.

Driver License, Vehicle Title, and Registration
Your driver license, vehicle title, and registration will require a certified marriage certificate and other identification.

Vehicle title name changes are handled at your county tax collector's office.

Banks and Credit Unions
Take your new driver’s license along with you to your bank.  Again, take your certified marriage certificate in case it is needed.

Social Security Administration

Read the Fact Sheet, the instructions, complete the application, and take all supporting documents, including the certified marriage certificate, to the local
Social Security Administration office.

Internal Revenue Service
The IRS has its own form for changing your name although it doesn't require any supporting documents.  Just print the form, complete it, and mail it to the IRS
office located in the instructions.   Although the form says 'Change of Address', Line 5 is where your old name is listed.

U.S. Passport Change
A certified marriage certificate must accompany the form from the link below to change a name on a U.S. Passport.  Which form to use depends on how long it
has been since the passport was issued.

U.S. Postal Service
If you are not moving but merely changing your name, just mention to your letter carrier that you got married so that any magazines, etc. coming in your
previous name should still be delivered, although your name has changed.

Voter's Registration
The Indian River County Supervisor of Elections provides a form on her website that can be completed, signed, and mailed to her office.

Credit Cards, Utilities, Finance Companies, etc.
These companies need to know of your name change so you won’t lose credit under your previous name and will still be able to obtain credit under your new
name.

Organizations and Memberships
Don't forget any clubs and associations you may be a member or organizations for which you may do volunteer work.  They probably won't require a marriage
certificate to change their rolls.

Other Legal Matters
Besides changing your name in your will, trusts, directives, safe deposit box, and investment holdings, it is also a good time to revisit beneficiary
designations.  And if you are a Notary Public, don't forget to change your commission.
